About The Role:
Port Lincoln Aboriginal Health Service is seeking a dedicated Registered Nurse to join our Chronic Diseases team, with the role commencing in the new year and continuing until 30 June 2027

Job Description
Key Responsibilities:
The appointee is responsible for the delivery of comprehensive primary health care services as part of the multidisciplinary team including:
Undertaking a wide range of clinical duties including but not limited to blood collection, wound dressings, patient screenings etc within well-established clinical pathways, protocols and guidelines
Contributing to the care planning and case management model in collaboration with other agencies to ensure best outcomes for the identified client group/s
Providing direct clinical support within care planning and case management
Liaising with Team Leaders/Program Managers to co-ordinate services with other personnel within a multi-disciplinary team environment and external partners and service providers.
Contributing to the facilitation of outreach services/support to Aboriginal people/groups unable to access services within the community environment (e.g., hospitals, aged care facilities, prisons etc)
Identifying and supporting health promotion and education opportunities for staff and clients as required by Team Leaders/Program Managers
Supporting ATSI people with chronic disease complex care needs to access a range of clinical and community support services which are tailored to meet individual needs.
Performing duties in regard to care coordination for clients enrolled in the Integrated Team Care programs, including:
Arranging required services outlined in the client’s care plan.
Ensuring arrangements for clients to attend appointments.
Assisting clients to participate in regular reviews by their primary care providers.
Supporting clients to develop chronic condition self-management skills.
Facilitating access to supplementary services and medical aids as per ITC guidelines. Delivering care coordination and supplementary services to ATSI clients with chronic conditions and to support local ATSI people make better use of available health care services, especially mainstream health services.
Undertaking other relevant duties as required across the service.

Demonstrated ability to work effectively in a multi-disciplinary team approach
Ability to communicate effectively with both Aboriginal and non-Aboriginal people in a culturally sensitive manner via an appropriate variety of methods
Ability to develop, keep and maintain accurate case files and client records
Demonstrated ability in using initiative, working independently with minimal supervision, determining priorities, meeting deadlines, planning and organising work.
Ability to effectively utilise information technology, data entry and Patient Information Recall Systems, in particular Communicare, and Microsoft Office products.
Proven clinical experience as a Registered Nurse in community setting/s
Experience in working effectively with Aboriginal and non-Aboriginal people.
Experience working in a multidisciplinary team to enhance service delivery through supporting staff expertise and morale
Knowledge of holistic Primary Health Care principles
Knowledge, understanding and awareness of the diversity of Aboriginal cultures and societies
Knowledge and understanding of Cultural Safety principles and their workplace implementation
Knowledge of Social Justice principles and practice
Knowledge of Equal Opportunity and Work Health & Safety (WHS) Legislation
Knowledge of relevant local resources and organisations
Knowledge of client-centred, consultative and community development approaches
Requirements:
Registered as a Nurse with the Australian Health Practitioners Registration Authority (AHPRA).
Competencies necessary to practise in the particular practice setting.
Minimum three years’ post graduate experience in a relevant field.
Must hold a valid, unrestricted driver’s licence
A current DCSI Working with Children Check (WWCC) issued within the last 5 years
A National Police Certificate issued within the last three years.
Evidence of compliance with PLAHS staff immunisation requirements including an annual influenza vaccination.
